All persons (except for an initial interview) need to provide all items on list A.

If coming for Candidacy Certification please also provide all items on list B.

18 collated copies of a recent sermon you have preached

A copy of any report or transcript for educational work completed since we last met

A completed mentor’s form (enclosed)

Written report from church supervisor or from local church pastor as to your involvement in the local church

18 copies of the following

Biographical form 102

Written response to statements in 311.3b

 

and any of the following documentation not previously supplied to the dCOM

 

 1 copy of the following documentation

Proof of membership in the recommending local church for two years

with S/PPRC and Charge Conference approval

Documentation of graduation from high school or its equivalent

 

It would also be helpful at this time to verify that dCOM has copies of any post-high school education i.e. Associates or Bachelors degrees.

1 copy of form 104  declaration of Candidacy for Ordained Ministry/Charge Conference Recommendation

Commitment to Highest Ideals of Christian Life form

Any other documentation requested by your mentor to be sent

 

In addition there is now a requirement to obtain a

Criminal and Credit Check documentation
