1. Autobiographical Statement: Personal and Vocational Update
Describe in several paragraphs the important developments in your life since your last interview. This may involve individual and
family matters, professional matters and/or spiritual matters. Your description should specifically include your spiritual growth and
formation as a disciple of Jesus Christ.
2. A Personal Affirmation of Faith: A one-page summary statement of what you believe as a Christian.
The Personal Affirmation of Faith is deliberately restricted to one page and should reflect your personal thoughts and beliefs.
3. A recent sermon that you have preached, both written and on CD or DVD (6 copies), and accompanying worship bulletin.
Please present the full written text of the sermon. This must be the same sermon submitted on CD or DVD. Include the worship
bulletin at the end of your sermon copy.
4. Disciplinary Questions: Paragraph 324 (2008 Discipline)
In your response to the disciplinary questions please be thorough in your discussion, paying attention to answering the question asked.
For 2012-2013 year please answer the following questions:
d.) How do you interpret the statement that Jesus Christ is Lord?
i.) Describe the mission and nature of the church. What are its primary tasks today?
p.) Explain the role and significance of the sacrements in the ministry to which you have been called.
5. A copy of any report or transcript for educational work completed since last interview (seminary classes or COS courses.)
6. A completed mentor's form signed by you and your mentor that reviews the last year of your meetings and conversations together.

7. A written report from your church supervisor or from your local church pastor describing your involvement in the local church.
PLEASE NOTE: All items listed above must be delivered to the Registrar's office no later than 12:00 noon on the second Wednesday
of the month, one month prior to your interview. For example: if your interview is scheduled for the month of October, your materials
must be to the Registrar's Office by the second Wednesday of September.
IF MATERIALS ARE NOT IN THE REGISTRAR'S OFFICE BY THIS DEADLINE, YOU WILL NOT BE INTERVIEWED FOR THE
YEAR. THIS WILL DELAY YOUR PROCESS AND MAY RESULT IN DISCONTINUANCE FROM THE PROCESS.
All materials should be proofread by your mentor or another Ordained Elder for content and for grammatical spelling, structure,
punctuation and typographical errors PRIOR to your submitting the material to the Registrar. Please do not rely on your computer's
spell-check. The United Methodist Church has always maintained high standards of excellence regarding educational qualifications and
these will be applied to your material.
