Web note: To save a copy to your computer, open the link by clicking on the form name. You should get a window that has three choices, Open, Save or Cancel. Click the Open button. This way you can see the actual form and know it is what you want. When page is open, go to the top of page click File, and save. You will then need to decide where to save it.
