All persons who wish to use our church facilities must fill out a "REQUEST FOR USE OF CHURCH FACILITIES" form and submit a signed form to the church for approval.  You can download this form to your computer in a Microsoft Document format ( .doc) or in  an Acrobat Reader format ( .pdf).
