Forms for use by our Conference Board
of Ordained Ministry, District Committees on Ministry
and Candidates for Ministry are available below. Most
are available in Microsoft Word format. Just click on
the appropriate form title to download. Once downloaded
you can fill out the form right on your computer by tabing
to the appropriate information cell. If the document doesn't
come up in Microsoft Word, right click on save it on your
computer and then bring it up in Word. Those forms
not available in Microsoft Word format you can download
as pdf files. If you don't have Acrobat
Reader you will need to download it
and install it on your computer.

The mission of the Church
is to make disciples of Jesus Christ." (Matthew 28: 19-20)
The annual conference is to make disciples for Jesus Christ
by equipping its local churches for ministry and by providing
a connection for ministry beyond the local church; all to the
glory of God. The Conference Board of Ordained Ministry is charged
with coordinating much of the formation and continuing formation
of the spiritual leaders who are licensed, consecrated, commissioned,
or ordained for the ministry within the Illinois Great Rivers
Conference. |
