Children's Forms:

Text Box: Every child needs a sponsor covenant form filled out by their sponsor (or parent if attending) AND a parental consent form filled out and both sent in with the child’s registration.   Text Box: Every youth must have a sponsor covenant form filled out by their sponsor (or parent if attending) AND a parental consent form filled out AND they need to fill out the covenant for youth (including the signature of your pastor) and send all in with their registration.
