- The files below are in "Portable Document Format" (that is, they are PDF's) and will need a PDF reader, at least, in order to read, complete, and print them. If you do not have such a reader, a free one is available by clicking on the icon below:
- Combined Pilgrim Application and Sponsor Form: The Pilgrim/Candidate should complete both sides of the form and sign it in the place shown, as well as his Sponsor and Pastor. It should then be mailed by the Sponsor, along with the fees for the Weekend (or partial payment with the completed Scholarship Request form below) to:Emmaus Registrar, PO Box 3624, Albuquerque, NM 87190. Please refer to the To Sponsor a Pilgrim page. Pilgrim Sponsors should complete the third page of the form and return it to the Registrar as directed on the form.
- Scholarship Application Form: Sponsors of Pilgrims, Team Members, or Clergy who need financial assistance for the Weekend should complete the form and return it as directed on the form. Sponsors should include it with their Pilgrim's application form found above.
- Clergy Availaility and Application for Emmaus/Chrysalis Team Form: Please download this PDF file and print it on your own printer, returning it to the Team Selection Committee as directed on the form.
- Northwest New Mexico Walk To Emmaus Survey and Volunteer Sign-Up Sheet: Please complete the form and submit it to the address on the form.
