To sign up for the Jr. High Spiritual Life Retreat, please fill out the following form and submit it. You may invite a friend if you'd like. Just let me know who you're inviting and have them fill out this form as well. Thanks!

Remember, you won't be officially signed up until you turn in your $50 registration fee to either Paul or the church office. You can make that check out to FUMC and put SHSLR at the bottom.

Since you're planning on going with us, please go ahead and fill out a medical form for the trip. If you click here, you'll be taken to a page where you can print out a medical form. Just fill it out, have your parent(s) sign it, then bring it with you and give it to me (Paul) before we leave on Saturday, April 15.
