RE:   Standardized Annual Lay Speaker Report form for 2005

TO:      District Directors of District Lay Speaker Committees

Attached is a form that you can use for the Annual Lay Speaker Report in your district.

Suggestions:

            Include the form in the District Church Charge Conference Packet

            Post the form to your District Web Site

            Email the form to your District Lay Speakers

            Email and discuss with your District Superintendent

 The Objective in the design of this report is to provide:

1) the administrative, contact, and approval information as required by the Book of Discipline.

 2) for input from the lay speaker as they reflect on their spiritual journey during the past year.

 3) for the communication of LS activities in mission and ministry and a focused plan of activity for the following year.

 A special thank you to all of you who helped to provide input in the final draft of this document

(the result of input from those who I had email contact information and were willing and able to provide input).

 Suggestions for next year are welcomed.
