In following the guidelines set by the State of Alabama for school children, a child enrolling in the CHUM Preschool program must be two, three- or four-years old by September 1 of the year of his/her enrollment.
 

All Forms required for enrollment should be completed and returned to the Preschool Office prior to the first day of attendance.  These forms include:

1. Registration Form  2. General Information Form  3. Emergency names/numbers  4. Transportation Plan  5. Medical Release Form  6. Blanket Field Trip Form*  7. Blue Immunization Record**

*The Blanket Field Trip Form for the 2 and 3-year-old Preschool children will allow them to go for walks to the empty lot next door to pick up fall leaves, acorns, etc. for art collages.

** THE BLUE IMMUNIZATION RECORD IS REQUIRED TO BE ON FILE AND MUST BE UPDATED WHEN NECESSARY.  Children's files are kept confidential and are kept in storage for a period of two years after the child leaves our program.

HOURS:  9:30 A.M. - 1:30 P.M.

 FEES AND TUITION:
A non-refundable registration fee of $100.00 is due at the time of enrollment.  Preschool tuition is based on a nine-month year at the following rates:

The Supply Fee is to be paid one time per year on ORIENTATION DAY in September.

POLICY CHANGES:
Policy may be changed at any time without prior notice.  All changes will be posted on the parent bulletin board at the Preschool office.  Notices of changes will also be sent home.
