  • Be a link to / between Pastor, Visiting Nurse, family members and other resources
    
  • Identify needs that will support as full a life as possible
    
  • Identify resources within the community and help facilitate contacts
    
  • Connect with Hospital Discharge Planner
    
  • Help people "frame" the appropriate questions
    
  • Be a supportive person and spiritual presence as part of a ministry team

  • Gather information to keep us all updated on health legislation and programs
    
  • Do homebound consultations / assessments
    
  • Support care givers
    
  • Visit the homebound
    
  • Answer questions
    
  • Review medications
    
  • Link people with support groups
    
  • Listen to your needs
