Registration/Financial Information

Registration Procedures:
1. Due to limited enrollment, you will want to register your child early to assure acceptance. If space is unavailable, we will be happy to put your child on a waiting list by completing the registration process that follows.
2. Registration is made by completing the Enrollment Form, Parent Agreement, Getting Acquainted Form, Discipline Form, Parent Notice and Biting Policy, then submitting a registration fee of $30 per child. The child's Physical Form must be turned in within two weeks after beginning at Bethel. Once you accept a spot for your child and we give you a date to begin, the registration fee is not refundable.
3. A waiting list is kept after enrollment reaches its limit of children. Children will then be accepted as space permits.
4. All checks should be made payable to Bethel Daycare Ministry. Cash should always be placed in an envelope marked with your name to ensure your account is properly credited.

Before and After School Cares:
Bethel Daycare Ministry only provides transportation for students who attend Washington Center Elementary School.
A Lincoln Elementary School bus has an established AM and PM stop at Bethel Daycare.
Also the Ft. Wayne Community magnet schools bus has an established PM stop at Bethel Daycare.
