Youth Scholarship Application

The Austin District United Methodist Men has a scholarship program for youth within the Austin District. The program is designed to give financial aid to youth seeking to attend civic youth-serving agencies and scouting ministries such as Boy Scouts, Girl Scouts, Camp Fire USA, 4-H Council, and other United Methodist Church supported events and activities (e.g., Methodist Youth Fellowship - MYF). The applicant will need a UMM sponsor from the church sponsoring the youth. Note: the applicant's information is confidential and will remain so throughout the application process.

Application Instructions

  1. Enter today's date
  2. Enter first and last name
  3. Enter the phone number where you can be reached in the evening
  4. Enter your mailing address. You MUST live in the Austin District of the United Methodist Church to qualify
  5. Enter your e-mail address if available
  6. Enter the name of the church that is sponsoring your youth group
  7. Enter your church's address with zip code
  8. Enter the name and phone number of the person sponsoring you from the UMM group
  9. Enter the youth group name / activity (e.g., "Troop 48 BSA" or "Crestview UMC MYF")
  10. Enter the date of the event / activity
  11. Enter the total per person charge for the event / activity for which you are requesting the scholarship. Note: Scholarships will be sent to the sponsoring UM Man who will make the funds available to you. Only UM sanctioned events / activities will be supported. All applicants would be allowed a maximum of 100% of the cost of tuition up to $200.
  12. Enter what the scholarship is to be used for. Specify location and dates of the event
  13. Enter a brief description of what type of activities you attend at your church and what responsibilities you have there
  14. Enter a brief description of what types of activities you do with your youth group and what responsibilities you have in your group
  15. Enter a short statement telling us about what getting the scholarship will mean to you.
