Please refer first to the Elders Forms List (Form 15) or the Deacons Forms Lists (Form 18) to familiarize yourself with the documents you will need for your particular ordination track. Please note, there are also documents listed below which are used by both Deacons and Elders. Those documents are listed in a separate section titled "Forms For Deacons and Elders." Please print out and use these online documents and not printed documents you may have collected previously. Those printed documents may be out of date!
