To obtain information regarding church business processes, forms to be submitted, or the status of activities in response to church business needs, use this web page.

Request Form - Availability
of this form through the website is for the convenience of those who
have access to the church website (and who don't get to the church campus
during the week; (blank forms are in Treasurer's mailbox), you do not
have to wait until you are at church to get a copy of the form, but could
have the form already filled out. This form must be used by anyone in
the church who is requesting reimbursement for an expense. Simply print
out the form, then fill it out and submit it to the Treasurer (by putting it in her mailbox at
church).
This
is an Excel spreadsheet and is a two-sided form. There are two worksheets in
the file, identified by the tabs at the bottom of the screen as follows
"2008 (front)" [front page] and "2008 (back) [back page]. Both sides of the form need to be completed.
